Senior Community Care of Maryland Provides for All Health Care and Social Needs of Seniors who Enroll in our Program.
Services are provided at our PACE Center, at home, and in other locations that match the needs of participants. Most PACE services, including medical care, therapies, and social activities, are provided at the PACE Center, ensuring coordinated, comprehensive care in one location. Our organization does not discriminate in the delivery of services based on race, ethnicity, national origin, religion, sex, sexual orientation, age, mental or physical disabilities or source of payment.
PACE participants must receive all needed health care services, including primary care and specialist physician services (other than emergency services), from Senior CommUnity Care of Maryland or an entity authorized by Senior CommUnity Care of Maryland.
DISCLAIMER: All services are based on need and offered after assessment by the Senior CommUnity Care PACE Interdisciplinary Team. PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
Adult Day Health Center
- A location to enjoy activities, meals and socialization. Bingo, crafts, trivia, walking club, board games, worship services and planned events are scheduled throughout the month.
- Weekly fitness classes to maintain strength and function. Classes on topics such as healthy cooking, mindfulness, bible study, and more. Quiet areas to rest and relax. Showers, personal grooming, and laundry areas. Nutrition support is provided by our dietitians at the PACE Center.
Transportation Services
- Transportation services to PACE Center and to specialty medical appointments. Drivers provide safe oversight for participants during loading and unloading.
- Drivers are CPR and first aid trained and must pass a drug screening and background check.
Medical & Specialty Care
- A primary care clinic with exam rooms and a triage area at the PACE Center.
- Primary Care services provided by our primary care providers.
- Nursing Care provided in the PACE clinic. On-call services are available 24 hours per day, 365 days per year.
- Specialty services including dental, vision, audiology, and podiatry as approved by the PACE organization.
- Lab work, X-rays, Surgery Emergency care, Hospital care.
- Prescription and over-the-counter medications are provided at no copay.
Interdisciplinary Team (IDT)
- Consists of Primary Care Provider, Registered Nurse, Social Worker, Physical Therapist, Occupational Therapist, Recreational Therapists, Registered Dietitian, Home Care Coordinator, Transportation Representative, Day Center staff member and Center Director.
- Coordinated care by the PACE Interdisciplinary Team provides a review of each participant’s care plan every six months, or more often if needed to ensure participant needs are addressed.
- The Interdisciplinary Team (IDT) is responsible for assessing each participant’s needs and determining the medical necessity of requested services.
Support with Meals
- Delicious, warm, and healthy meals are served on weekdays at the PACE center. Take-home delivered meals are also available if needed.
Therapy Options
- Therapy gym with parallel bars, exercise bikes, weights, and resistance machines to improve movement and safety at PACE Center.
- Physical therapy services to develop, maintain, and restore maximum movement and functional ability provided by licensed therapists at the PACE center.
- Occupational therapy services assist individuals to develop, recover and improve the skills needed for daily living at the PACE center.
- Home visits and modifications to ensure a safe living environment.
- Durable Medical Equipment such as canes, walkers, and wheelchairs.
Health & Personal Care Services
- Skilled home health services.
- Personal care such as bathing, dressing and medication assistance.
- Meal preparation.
- Light housekeeping.
Vision Care
- Vision services provided by a contracted provider in the community.
- Eyeglasses based on assessed need.
Dental Care
- Cleaning and preventative care by a Dental Hygienist.
- Dental work provided by a PACE dentist or contracted provider in the community.
- Dentures based on assessed need.
Hearing Care (Audiology)
- Hearing services provided by a contracted provider in the community.
- Hearing aids based on assessed need.
Podiatry
- Preventive and routine foot care.
Mental Health & Social Services
- Mental health services with contracted providers to enhance the independence and growth of our participants.
- Grief and support services.
- Social workers and case managers assist with the provision of additional resources as needed.
- Spiritual services provided by PACE Chaplain.
Palliative & End-of-Life Care
- Participant and family-centered care that is focused on improving quality of life and relieving suffering in the last years, months, weeks or days of life.
- Care addresses physical, psychological, social and spiritual needs.
- Care provided in your home or community setting by the PACE team.
- Respectful of individual autonomy, self-determination, and personal wishes.
